New Member Process
Referral sources or potential members can download a Referral Form.
Once completed, please send the form to Program Director Earlene DeYoung via email, mail, or fax. Call ahead before faxing – our machine is public.
After the Referral Form has been received, an Intake Coordinator will reach out to schedule a tour and your orientation.
